•After oropharyngeal cancer surgery, long-term quality of life is well-preserved.•Main symptoms are fatigue, reduced sexual activity and oral function-related disorders.•At long term, more than one third of patients present an anxiodepressive disorder.•Half the patients who are still working at the time of diagnosis will return to work.•Psychological distress is the main determinant of long-term quality of life.
