•To examine the potential mediating effects of health behavior between type D and vulnerable plaque in CAD patients.•The vulnerable plaque was assessed in vivo by optical coherence tomography.•Results showed health behavior (psychological stress and living habits) mediate the association between type D and vulnerable plaque.
