•We identified gradient (Grad) as the best indicator for estimating flow conditions in ICG-VAG.•Grad and actual flow rate were exponentially correlated both in vitro and in vivo.•Grad was calculated using FlowInsight within 3 minutes intraoperatively.•Elevation of Grad indicated postoperative hyperperfusion in ECA–ICA bypass surgery.
