Preoperative imaging and microscopic navigation during surgery can avoid unnecessarily opening the mastoid air cells through craniotomy using the retrosigmoid approach.

•Anatomic structure of bone and vessel can be more clearly displayed by image registration and fusion.•The transverse and sigmoid sinuses junction can be accurately found under microscopic navigation.•Test and control groups showed significant differences in the incidences of opening MACs.
