•NPC survivors treated with IMRT demonstrate moderate rates of neurocognitive impairment.•Observed patient and family reported neurobehavioral dysfunction in this cohort was high.•Further work is needed to reduce neurocognitive and neurobehavioral dysfunction is needed.
