•There are limited oral add-on therapies to statins to obtain LDL-C goals.•Gemcabene may offer a novel mechanism for LDL-C lowering.•Gemcabene add-on produced significant LDL-C lowering > 20% compared to placebo.•Gemcabene add-on produced significant CRP lowering > 40% compared to placebo.•Data support development of gemcabene for patients needing further LDL-C lowering.
