Implementation of Client Incentives within a Recovery Navigation Program

•Study goal was to support post-detoxification recovery and reduce health care costs.•Participants received recovery navigation services with or without incentives.•Delivery and uptake of incentives were low; 49% earned any incentives.•Health care cost increased from pre- to post-detox and was not reduced with incentives.•Barriers to effective incentive implementation are discussed.
