•Low-grade ductal carcinoma in situ (DCIS) shows much greater variability in interpretation than high-grade DCIS.•A reference standard of low-grade DCIS was almost as likely to be interpreted as a higher diagnostic category than as a lower one.•Clinical management of DCIS may need to account for grade by distinguishing low-from high-grade DCIS.
