APOLIPOROTEIN E POLYMORPHISM IN CHINESE POPULATION WITH VARIOUS TYPES OF COGNITIVE DISORDERS

Abstract
The association between ApoE and various forms of cognitive disorders remains controversial. We are investigating the ApoE polymorphism in Chinese population: 1. among neurodegenerative dementia (ND), mild cognitive impairment (MCI), subjective cognitive dysfunction (SCD) and normal control; 2. among AD, FTD, LBD, leukoencephalopathy with dementia (LwD) and cerebral amyloid angiopathy (CAA). Besides, we are trying to explore the relationship between sex, educational attainment, handedness and cognitive decline. 3622 participants were enrolled in the baseline survey between 2007 and 2016 from Peking Union Medical College Hospital, China. The whole sample was divided into two groups: 1. Patient group (n=1269): with the existence of complaints suggestive of cognitive impairment. 2. Control group (n=2353): without complaints suggestive of cognitive impairment. 896 participants in patient group were excluded because of missing genotype, cognition, or etiological examination. Finally, data on 373 individuals in patient group were included. All the subjects finished history inquiry, neurological examination and ApoE genotyping. Besides, the patient group underwent etiological and neuropsychological tests (MMSE, Moca, ADLs and HADS). Some were further evaluated by neuropsychological domain assessment battery, comprising of executive, visuospatial, memory, language, reasoning and judgement functions. The patient group were subdivided into three subgroups: dementia, MCI and SCD. The dementia group were further divided into six categories: AD, FTD, LBD, LwD, CAA and other dementias. The main results were shown in Figures 1–3. 1. ND, MCI and SCD subgroups have a significantly higher proportion in ApoE-ε4 carriers than the controls. This indicates that ApoE-ε4 genotype might have an effect on the initiation or progression of cognitive decline. However, there is no difference among SCD, ND and MCI patients. Follow-ups are advised for SCD patients. 2. AD and CAA patients have a significantly higher proportion in ApoE-ε4 carriers than other neurodegenerative dementias. 3. ND patients have a significantly lower level of educational attainment than MCI and SCD patients. We are wondering whether improving the educational level and cognitive reserve could slow down the progression of cognitive decline.
