摘要
Health Nutrition and Examination Survey 2013-2016.There were 14,780 persons who participated in the serology testing for hepatitis B and C. We defined HBV past exposure by the presence of anti-HBc, chronic HBV infection by HbsAg positivity, and HCV infection by HCV RNA positivity.Significant fibrosis was determined by AST to Platelet Ratio Index (APRI) score >0.7.Awareness of viral hepatitis was determined by a question: "Have you ever told you had hepatitis B or C?" For all analyses, we used published weights to account for oversampling and nonparticipation. Results: There were 885 persons with HBV past exposure (weighted prevalence: 4.1% [95% CI: 3.5-4.6]),71 persons with HBV chronic infection (0.3% [95% CI: 0.2-0.4]),and 117 persons with HCV infection (0.7% [95% CI: 0.6-0.9]).The awareness rate of HBV past exposure, chronic infection, and HCV infection was 11.7% [95% CI: 7.8-15.5],32.4% [95% CI: 15.7-49.1],and 55.6% [95% CI: 42.6-68.6],respectively.By age group, awareness rate of HBV past exposure, chronic infection, and HCV infection was 9.9%, 24.4%, 49.5% in aged 20-39, 16.0%, 45.3%, 57.0% in aged 40-59, 8.2%, 29.5%, 55.0% in aged $60, respectively.61.5% [95% CI: 46.2-76.7] of HCV infected baby boomers (born between 1945 and 1965) were aware of their infection.By gender, the rate was 13.8%, 29.4%, 54.9% in male and 8.9%, 36.0%, and 57.4% in female.By race, the awareness of HBV past exposure was 20.3% in Whites, 4.6% in Blacks, 7.6% in Hispanics, 8.7% in Asians and 11.0% in other race.Awareness of HBV chronic infection was 32.3% in non-Asians, non-Blacks, 39.2% in Asians, and 13.9% in Blacks.Awareness of HCV infection was 30.6% in Whites, 57.3% in Blacks, 45.9% in Hispanics, and 60.9% in Asians or other race.Lastly, awareness rate of the disease among persons with significant fibrosis and elevated liver enzymes was 62.0% in chronic HBV infection and 39.3% in HCV infection. Conclusion: The current awareness rate of viral hepatitis B and C infection in the United States is suboptimal.More sophisticated public health policy to identify persons at risk and provide appropriate management is warranted.
