•Pre-treatment metabolic tumour volume >35 cm3 predicts worse PFS and CSS in anal cancer.•Higher 6-month-post-treatment maximum and peak standardized uptake value predict worse PFS and OS.•Further study is warranted evaluating dose escalation for tumours with higher pre-treatment MTV.
