•In critically ill, obese, mechanically ventilated patients, 50% had resting energy expenditure successfully measured by indirect calorimetry.•Compared with indirect calorimetry, ASPEN equations underestimated energy need for 88% of patients.•Actual energy intake was lower than 65% to 70% measured REE for 96% of patients.
