•Thirteen metals in urine and platelet counts in blood were repeatedly measured in three trimesters.•Manganese, arsenic, and cadmium were associated with decreased platelet counts during pregnancy.•Cadmium exposure increased the likelihood of gestational thrombocytopenia.
