•More than one fifth of PC patients have an identified cardiac anomaly.•Nearly one fifth of PC patients report chest pain to their physician.•Symptoms are not predictive of structural abnormalities.•PC patients have notable rates of mitral valve prolapse and aortic root dilation.•Echo is recommended for all patients with PC regardless of symptoms.
