•It is uncommon for people with MRI/PET-negative drug resistant focal epilepsy to undergo surgical resection of epileptic foci.•With careful planning and patient selection, patients with MRI/PET-negative drug resistant focal epilepsy can achieve favorable long-term surgical outcomes.•FCD1a is a common pathology found in MET/PET-negative patients with drug resistant focal epilepsy.
