•In the real-world data, 4/5 of NVAF patients received recommended doses of DOAC.•Patients with low dose of DOAC had worse clinical outcomes.•Appropriate dose reduction was associated with decrease of bleeding events.•Appropriate dose reduction did not impair efficacy endpoints.•Compliance with the standard criteria is of paramount importance.
