Utility of fenestrated straight clips in microsurgical clip reconstruction of an atherosclerotic middle cerebral artery aneurysm extensively involving two efferent arteries: Technical case report

•Clipping of an atherosclerotic MCA aneurysm may have led to a remnant neck.•A fenestrated clip can be placed closer to the neck than a nonfenestrated clip.•A fenestrated clip can be applied to reduce a remnant aneurysm.•Parallel clipping with fenestrated clips is more effective compared to nonfenestrated clips.
