•CSF lactate level was significantly higher in AQP4-Ab positive NMOSD patients comparing with MS.•CSF lactate level may be helpful for distinguishing AQP4-Ab NMOSD from MS.•The increased CSF lactate level may contribute to the disease progress mediated by AQP4-Ab in NMOSD.
