•Eversion endarterectomy of the external iliac artery (EEEIA) could be an autologous alternative technique for patients with chronic limb-threatening ischemia in high risk of prosthesis infection.•Rare surgical complications, and high rates of patency and amputation-free survival were observed.•EEEIA is a safe and effective autologous technique to treat extensive iliofemoral obstructions.
