•Modern open-heart surgery induces minimal lung edema in neonates.•Stress-dose corticosteroid regimen reduced radiographically assessed lung edema.•Stress-dose corticosteroid improved postoperative dynamic lung compliance.•No benefit for need of postoperative ventilator support or oxygenation was observed.•No benefit for inflammatory response in tracheal aspirate was observed.
