•Denmark introduced a National Clinical Guideline development programme in 2012.•Development of Clinical Guidelines was centralized, following the GRADE methodology.•Guidelines were developed primarily for areas with uncertainty about best practice.•Most guidelines provided weak recommendations for practice.•Because recommendations are weak and without systematic monitoring of compliance, the effect on reducing practice variation is uncertain.
