Temporal lobe epilepsy: A never-ending story

•Ictal semiology remains invaluable in the presurgical study of patients with temporal lobe epilepsy.•Forty six patients Engel Ia and 16 Engel II-IV with a median postsurgical follow-up of 5 years were included.•Classical semiology prevailed in Engel Ia, but some infrequent signs were well-characterized.•Infrequent ictal signs in Engel Ia: maintenance of responsiveness, language disturbance, and RINCH.•Only in Engel II–IV: inferior limbs stepping-like automatisms and postictal dysarthria.
