•The SARS-CoV-2 pandemic has caused the collapse of health systems across the globe.•Acute appendicitis is the most common emergency general surgery pathology.•Surgical management of appendicitis and diagnosis of SARS-CoV-2 is feasible.•In low-income countries, we recommend early appendectomy for SARS-Cov-2 patients.•Recommend cautious generalization of reports on higher perioperative mortality.
