A hepatitis C elimination model in healthcare for the homeless organization: A novel reflexive laboratory algorithm and equity assessment

•We created a novel laboratory bundle combining screening and treatment evaluation for hepatitis C.•Multinomial logistic regression assessed contributors to care cascade progression and equity.•The lab bundle increased hepatitis C care cascade progression and decreased time to treatment.•Women and homeless individuals were less likely to complete labs confirming hepatitis C cure.•There was equitable screening and treatment initiation but low confirmation of cure rates.
