•We examined the theory that glial activation is associated with 9/11 exposures.•TSPO-Vt was examined using PET in 20 responders adjusting for TSPO genotype.•Responders with PTSD had increased TSPO distribution volume in the hippocampus.•Heavily exposed responders had increased TSPO distribution in the parietal cortex.
