•In eyes with neovascular age-related macular degeneration, a change in central subfield thickness is a poor surrogate for visual acuity outcomes.•The findings from this study are similar to findings from diabetic macular edema.•Central subfield thickness changes after anti-vascular endothelial growth factor for neovascular age-related macular degeneration reflect favorable and unfavorable visual acuity outcomes.•Neither agent nor dosing regimen explained much of the variance in visual acuity outcomes.
