The effects of cannabis use on physical health: A co-twin control study

•We observed phenotypic associations between cannabis and exercise, heart rate, and appetite loss.•Families with greater cannabis use exercised less and had more frequent appetite loss.•Within-twin pair differences in cannabis use were associated with a lower heart rate.•Results suggest that cannabis use is not associated with detrimental physical health effects.
