•Women in cardiothoracic anesthesiology experience many professional challenges.•Career advancement, compensation, promotion, and authorship were affected by gender.•Having children was perceived to have a negative effect on career advancement.•Experiences with microaggressions and intimidation were pervasive.•Systemic changes are needed to address these professional challenges.
