•The skeletal muscle wasting (SMW) during neoadjuvant therapy (NAT) is quantified.•SMW during NAT is a novel prognosticator different form sarcopenia for poor survival .•Neoadjuvant chemoradiotherapy and old age are risk factors for severe SMW during NAT .•To maintain skeletal muscle during NAT could potentially promote therapeutic outcomes .
