•Increased disability in NMOSD is related to lower quality of life (QoL).•Pain/discomfort and mobility were the most highly affected QoL domains.•NMOSD relapse/attack prevention is key to improving QoL outcomes.•Results support the use of the EDSS in NMOSD.
