PS 17-65 THE C344T GENE POLYMORPHISM OF ALDOSTERONE SYNTHASE IS RISK FACTOR OF EARLY RENAL DAMAGE IN HYPERTENSION IN A CHINESE HAN POPULATION

Objective: This study used the case-control method to investigate the association between the C344T gene polymorphism of aldosterone synthase and early renal damage in a hypertensive Chinese Han population. Design and method: A total of 418 adult essential hypertensive patients were selected as the study subjects. The personal information, clinical indicators (blood lipids, blood glucose, aldosterone and other indicators) and the C344T gene polymorphism of aldosterone synthase were texted. This study used the urine microalbumin/ urine creatinine ratio as a grouping basis: the hypertension without renal damage group (NRD group, n = 236) and the hypertension combined with early renal damage group (ERD group, n = 182). Results: (1) Compared with the NRD group, the ERD group had, a longer hypertension history (9.61 ± 9.39 years, 13.29 ± 10.34 years, P < 0.05), and increased systolic pressure (164.34 ± 26.42 mmHg, 187.25 ± 30.56 mmHg, P < 0.01), diastolic pressure (97.23 ± 17.27 mmHg, 106.47 ± 24.98 mmHg, P < 0.05), and aldosterone levels (241.13 ± 116.09 pg/ml, 299.76 ± 137.99 pg/ml, P < 0.05). (2) There were significant differences in genotype frequency and allele frequency distribution between ERD and NRD group. The TT genotype and the T allele frequency in ERD group was higher than those in NRD group (P < 0.05). (3) The aldosterone level (264.83 ± 125.57 pg/ml, 208.77 ± 88.06 pg/ml, P < 0.05) in the TT genotype group were higher than those in the CC genotype group; The systolic pressure (173.70 ± 25.28 mmHg, 164.63 ± 30.70 mmHg, 161.08 ± 33.58 mmHg, P < 0.05) in the TT genotype group were higher than those in the CT and CC genotype group; The diastolic pressure (100.56 ± 19.49 mmHg, 98.68 ± 18.78 mmHg, 89.27 ± 16.02 mmHg, P < 0.05) in the CT and TT genotype group were higher than those in the CC genotype group. Conclusions: Early renal damage in the Chinese hypertensive population was associated with systolic pressure, diastolic pressure and aldosterone. The C344T gene polymorphism of aldosterone synthase might be risk factor of early renal damage in the hypertensive Chinese Han population.
