•Laparoscopic omental cerebral transposition is an effective and safe revascularization procedure for Moyamoya disease in both the pediatric and adult populations.•Considering the favorable properties of omentum and the feasibility of the laparoscopic harvesting approach, this technique might be applied for any ischemic condition requiring extraperitoneal revascularization.
