Radiological Study of the Posterior Septum—Nasal Floor Mucosal Flap

Objective: This study captured the dimensions of the first described posterior septum—nasal floor (PSNF) mucosal flap through radiographic study. Endoscopic endonasal approaches to the skull base is a rapidly evolving area of surgery where innovation in reconstruction and approaches to defects provides greater opportunity of resecting complex pathology. Currently, the workhorse flap used for skull base reconstruction is the nasoseptal flap (NSF). Although the NSF is partly responsible for increasing both the breadth and feasibility of endoscopic resection of the skull base tumors, the flap harvest carries morbidity that can adversely impact quality of life. With the demonstrated success decreasing postoperative sinonasal morbidity with the nasal floor free mucosal graft, this harvest site could feasibly undergo transformation into a pedicled flap, harnessing the robust blood supply of the sphenopalatine artery (SPA) while avoiding the morbidity that comes with the NSF.
