•Fistulas after sleeve gastrectomy are complex and have multifactorial origins.•Indocyanine Green (ICG) is a liquid that can be injected into the human bloodstream without adverse effects.•This is a case report of a 40-year-old female with a positive intraoperative indocyanine green test as a prediction of the leak after sleeve gastrectomy.
