Acompanhamento clínico farmacêutico no cuidado ao paciente adulto-cirúrgico em um hospital universitário de Porto Alegre

Introduction: Although the clinical pharmacist is already well established in some institutions and health services have this professional in their teams, few are able to systematize their work and map the activities developed data, demonstrating the relevance of this professional in the health team. The aim of this study was to evaluate the clinical pharmaceutical follow-up in an adult-surgical inpatient unit in a university hospital in Porto Alegre. Methods: Retrospective descriptive study, which quantified the main activities of the clinical pharmacist in an adult surgical inpatient unit from January to May 2019. The Research Ethics Committee of that institution approved this project. Results: 859 patients were admitted to the evaluated surgical unit, of which 490 were reviewed on hospital admission, corresponding to the average rate of 57.27%. The average rate of drug reconciliation performed by the clinical pharmacist in this unit was 14.83%, totaling 73 patients reconciled per interview. 361 pharmaceutical interventions were performed during the study period, 54 of which were related to medication reconciliation, and the total number of adhesions was 232. The main surgical specialties that hospitalize patients in the unit in question were Orthopedics, Surgery of the Digestive System, Urology and Vascular Surgery. Conclusion: It was possible to evaluate the clinical pharmaceutical follow-up in an adult-surgical inpatient unit in a university hospital in Porto Alegre, through the quantification of the rates of reviewed patients and medication reconciliation, the number of pharmaceutical interventions and their adherences, in addition to characterize the main surgical medical specialties involved.
