What Data from Nutrition Counseling and Therapy need to be collected and how do they get into a Hospital's Discharge Letter?

The structured documentation of dietary data for inclusion in mandatory discharge management is a challenge for hospitals. A documentation concept was developed for the data to be documented in nutritional counseling and therapy, taking process-guided work into account, and compared with practice in a case study. The target-performance comparison shows that due to deficient structural and personnel resources as well as a lack of implementation strategy, documentation is not very structured and not complete. Necessary adjustments and recommendations for action were identified. With the documentation concept, a decisive step has been taken toward expanding the discharge letter to include diatetic data. Further studies on practical transfer are required.
