•Time in therapeutic range >65% is recommended in adults with a congenital heart defect.•A low rate of thromboembolic and major bleeding was found in ACHD with good anticoagulation therapy.•Low rate of thromboembolic events and major bleeding events for low-moderate risk ACHD patients with good quality of VKA anticoagulation.
