Development of a Transition of Care Intervention for Acute Coronary Syndrome to Increase Continuity of Care and Uptake of Cardiac Rehabilitation: A Co-Design Study

Attendance and completion of outpatient cardiac rehabilitation (CR) is essential for recovery and long-term management of cardiovascular disease, however only ∼30% of eligible patients attend. Reasons for low attendance include lack of awareness, low referral rates and fragmented systems of care. We aimed to co-design a digital intervention to increase continuity of care for patients after hospital discharge and increase uptake of CR.
