Quand les cellules fusiformes se réarrangent…

Nous rapportons le cas d’une femme âgée de 79 ans pris en charge en service de chirurgie digestive pour un syndrome occlusif. Le scanner abdominopelvien met en évidence une volumineuse masse pelvienne infiltrant l’utérus, la vessie et le côlon. Une exérèse monobloc est effectuée. L’examen microscopique montre une infiltration de la paroi de ces organes par des glandes revêtues d’un épithélium mucosécrétant bien différencié sans atypies dont les cellules expriment la kératine 7 et l’antigène carcinoembryonnaire, sans marquage avec les anticorps anti-p16, récepteurs hormonaux et kératine 20. L’index de prolifération est faible. Le diagnostic d’adénocarcinome endocervical mucineux de type gastrique à déviation minime est posé. Cette tumeur rare représente environ 1 % des adénocarcinomes endocervicaux. Il s’agit d’une tumeur de diagnostic histopathologique difficile devant être connue des pathologistes. Elle est grevée d’un mauvais pronostic avec de fréquentes localisations ganglionnaires ou extra-utérines au diagnostic, comme l’illustre notre cas.We report the case of a 79 year-old woman, who was admitted in the department of digestive surgery for an occlusive syndrome. The abdominopelvic computed tomography revealed a voluminous mass infiltrating the uterus, the bladder and the colon. A monobloc surgery is performed. The histopathological examination evidences an invasion of these organs by a well-differentiated glandular proliferation composed of epithelial cells without atypia stained with keratin 7 and carcinoembryonar antigen, without expression of p16, hormonal receptors and keratin 20. Ki-67 labeling index was low. The diagnosis of minimal deviation gastric mucinous adenocarcinoma was proposed. This is an uncommon neoplasm comprising approximately 1% of all endocervical adenocarcinomas. This is a difficult histopathological which should be known by pathologist. It is correlated to a poor clinical outcome with a high tendency to lymph node and extra-uterine metastases, as illustrated in our observation.
