•Oxidative stress generated RNA damage seems to be a trait factor of BD.•Persistent increase in levels of oxidative stress over time in patients with BD.•Persistent increase in oxidative stress levels in unaffected relatives compared with HC.•No state alterations in oxidative stress generated nucleoside damage were found.•Illness load did not impact levels of oxidative stress generated nucleoside damage.
