•This is the first detailed phenotype of visual outcomes in people with idiopathic intracranial hypertension that required cerebrospinal fluid diversion surgery for visual failure.•The modeled trajectories show that optical coherence tomography imaging markers of papilledema and visual field mean deviation reached maximal recovery by 4 months.•Macular ganglion cell layer loss continued for ≤12 months after surgery.Headache symptoms persisted for many patients despite normalization of intracranial pressure.
