•New-onset hypertensive disorders of pregnancy (HDP) increased in Maine since 2007.•HDP pregnancies in 2007–2019 were examined for sequalae in the first year postpartum.•Acute care encounters were similar by rurality of residence.•Antihypertensive medication prescriptions were also similar by rurality.•However, risk of these outcomes was common and varied according to HDP subtype.
