•We included 2552 patients, of which 673 inlay and 1879 onlay PFAs.•No significant differences in KSS and VAS between inlay and onlay PFA were found.•The WOMAC score was superior for the inlay group.•There were higher rates of conversion to TKA after inlay compared to onlay PFA.
