CRT-700.55 Left Atrial Appendage Occlusion With Watchman® Device in Patients With Non-Valvular Atrial Fibrillation and End-Stage Chronic Kidney Disease on Hemodialysis (EPIC06 - WATCH-HD)

Two patients with severe aortic paravalvular leaks (PVLs) in a mechanical prosthesis were percutaneously treated. Procedures were performed with mild sedation, angiographic guidance, and a 6-French right radial artery approach with good final results. Percutaneous PVL treatment is routinely performed with general anaesthesia, transesophageal echocardiographic guidance, and femoral access. Although another case of radial aortic PVL closure has been previously reported, it was performed with an 8-French radial sheath, transesophageal echocardiographic guidance, and general anaesthesia. According to our initial experience, aortic PVLs could be treated in a minimally invasive way. Larger studies should be conducted to confirm these observations.Deux patients munis d’une prothèse mécanique présentant de graves fuites paravalvulaires aortiques ont été traités avec succès par voie percutanée. Les interventions, effectuées sous sédation légère, ont fait appel à l’angiographie pour guider un cathéter (taille 6-Fr) qui avait été introduit par l’artère radiale droite et ont donné des résultats favorables. Ce type d’intervention est habituellement réalisé sous anesthésie générale, par voie fémorale et guidé par échographie transœsophagienne. Un seul autre cas de réparation de fuites paravalvulaires aortiques par voie radiale avait été répertorié jusqu’ici, mais on avait alors procédé à l’intervention à l’aide d’un cathéter de taille 8-Fr guidé par échographie transœsophagienne chez un patient sous anesthésie générale. Si on se fie aux résultats obtenus, il serait possible de traiter les fuites paravalvulaires de manière minimalement invasive, mais d’autres études de plus grande envergure seront nécessaires pour confirmer ces premières observations.
