•miR-448-5p is poorly expressed in the mouse model of ALI.•XIST exacerbates ALI in mice by inhibiting expression of miR-448-5p.•miR-448-5p inhibits HMGB2 expression.•XIST by monocyte-derived Exos regulates miR-448-5p/HMGB2 to promote ALI.•This study provides novel insights into the mechanism of ALI progression.
