•A novel 3.5-day primaquine (PQ) regimen was well tolerated and relatively safe.•Supervised doses given with food ameliorated the gastrointestinal adverse effects.•Pharmacokinetic data support the ultra-short PQ regimens with a twice-daily dosing.•Short-course PQ regimens will likely improve treatment acceptability and adherence.
