0.58 vs. 0.25; p=0.01), averaged longer time from first SPC encounter to death (mean § Standard Deviation (SD) days: 134 §32 vs. 102 §57; p=0.03), and averaged higher SPC physician full-time equivalents (mean §SD: 1.28 §0.95 vs. 0.46 §0.60; p=0.05) relative to low-reach VAMCs.There were linear trends increasing from bottom to top quintiles for time from SPC to death and a potential threshold effect for physician full-time equivalents.Conclusions.VAMCs have wide-ranging rates of SPC for persons with aHF.High-reach VAMCs provide more outpatient care, have earlier referrals to SPC, and report higher physician staffing than low-reach VAMCs.Implications.Research is needed to better understand SPC delivery and implementation in high-and low-reach VAMCs for persons with aHF, compare outcomes, and identify and disseminate best practices in SPC delivery.
