Pre-surgical features of intrinsic brain networks predict single and joint epilepsy surgery outcomes

•Identifies patient-specific features of intrinsic connectivity networks (ICNs) that predict outcomes after epilepsy surgery.•First study to report on the brain functional and structural features that serve as substrates for multiple types of joint outcomes following epilepsy surgery.•Demonstrates that good joint seizure and cognitive/psychiatric surgical outcomes depend upon higher levels of structural brain reserve in select ICNs.•Demonstrates that singular outcomes rely on indices of systematic, idiosyncratic variance in the resting state signal in the case of seizure control. In contrast, singular cognitive/psychiatric outcomes depend on weakened pre-surgical representations of select ICNs.•Demonstrates that ICNs differ in their propensity to provide the structural (brain) or functional (cognitive) reserve that supports good post-surgical outcomes.
