•Osteosarcomas of the head and neck are rare and recurrence are mainly local.•Surgery is the principal therapeutic modality for resectable head and neck osteosarcomas, however, chemotherapy and/or radiotherapy are being considered for patients with high-grade lesions, pathological adverse features or close margins resection.•Surgical resection with negative resection margins may improve survival, and a 3 mm resection margin threshold may optimize survival.
