•Isolated hypoglossal nerve palsy (HNP) is a rare condition.•Causes of isolated HNP include tumours, trauma, stroke, surgery and infections.•Isolated HNP typically resolves spontaneously within months.•Extended work-up is necessary to rule out reversible and treatable causes of HNP.•This case report highlights a potential link between tongue angioedema and HNP.
